A new report from Scottish Government and COSLA has been published which outlines measures to reduce delayed discharge and provide care closer to home for people with learning disabilities and complex needs. Actions from the Coming Home Implementation report include setting up a new national register to improve monitoring of those at risk of hospital admission or inappropriate placements. 

The report also recommends  a new national panel and a peer support network to bring together people who provide services and help for people with learning disabilities and complex needs to share expertise and ensure genuine change is delivered. You can read the report in full here.
